This form is used to request a generic email account for your school or department. Before granting a new generic account, existing ones will be reviewed.

Below you will list the purpose/function for needing the account, the generic name and requested email address (subject to change based on ITM review) and the owner.

The owner of the account will be responsible for managing the account, including delegating access to others.

Upon completion ITM, will send the login information to the owner.
